Notice of Privacy Practices (HIPAA)
Your privacy and confidentiality are very important to us. As required by federal law (HIPAA), this Notice describes how Calm Place Therapy may use and disclose your protected health information (PHI), your rights regarding your health records, and our responsibilities as your mental health provider.
Your PHI may be used for treatment, billing, healthcare operations, or when required by law (such as mandated reporting or emergencies). Outside of these exceptions, we will only release your information with your written consent.
You have the right to:
- Access and obtain a copy of your records
- Request corrections to your records
- Request restrictions on certain uses or disclosures
- File a complaint if you believe your privacy rights have been violated
