I am an out of network provider. I am happy to provide you with a superbill to send to your insurance to apply toward you out of network deductible. I suggest calling your insurance company and asking the following questions about your out of network coverage.
1. Do I have mental health benefits?
2. What is my deductible?
3. Once my deductible is met, what percentage is reimbursed?
4. What is the turnaround time for getting my reimbursement?
If you cannot keep the scheduled appointment, you must notify our office to cancel or reschedule the appointment within 24 hours of the scheduled appointment time. If the session is not canceled within 24 hours, a full session rate will be charged for the missed appointment.
